Remote Functional Capacity Evaluation

• 31 year old male.

• Struck by an automobile while leaving a bar.

• Resulted in a Fractured Pelvis and Low Back Sprain.

• Treated for 2 years and declared to be totally disabled for life by his Orthopedic Surgeon.
